Benefit Eligibility Summary
$500 Deductible Plan  for  Health, Dental, Vision Insurance OR

$2500 Deductible Health Savings  Account (HSA) for Health, Dental, Vision Insurance

All full-time employees of V-Soft are eligible to participate in any one of these plans from his/her hire date. Provided through Anthem BCBS. $500 Deductible Plan, 80% after deductible with office co-pay and drug card. Employees who do elect participation in the health plan are subject to the rules and guidelines associated with the health insurance carrier. Provided through Anthem BCBS. $2500 Deductible Health Savings account plan of benefits. This allows employees to put $3,100 (Single) or $6,250 (Family) in a qualified tax free savings account through CHASE. The payroll deduction is less and the funds you deposit in the account are pre–taxed.
Life Insurance & AD&D All full-time employees of V-Soft are eligible to participate in Life insurance from his/her hire date. Company paid. Provided through Anthem Life. Maximum benefit up to $50,000. Accidental Death benefit included-pays and additional $50,000.
Voluntary Life All full-time employees of V-Soft are eligible to participate in Voluntary Life insurance from his/her hire date. Anthem Voluntary Life is an additional benefit up to $500,000. You have an option to enroll or decline this coverage. If you enroll, premium deductions would be based on your age and coverage amount.
Voluntary Spouse Life All full-time employees of V-Soft are eligible to enroll their spouse in to Voluntary spouse Life insurance from his/her hire date. Anthem Voluntary Spouse Life is an additional benefit up to $250,000. You have an option to enroll or decline this coverage. This is contingent upon employee’s enrollment into Voluntary Life. If you enroll, premium deductions would be based on age and coverage amount.
Voluntary Child Life All full-time employees of V-Soft are eligible to enroll their child/children in to Voluntary Child Life insurance from his/her hire date. Anthem Voluntary Child Life is an additional benefit up to $10,000. You have an option to enroll or decline this coverage. This is contingent upon employee’s enrollment into Voluntary Life. If you enroll, premium deductions would be based on age and coverage amount.
Short-Term Disability (STD) All full-time employees of V-Soft are eligible to participate in Short-Term Disability insurance from his/her hire date. Company paid. Provided through Anthem Life. Pays 60% of your pay up to $1000 per week on the 1st day of an accident or after 8th day of sickness. Must exhaust PTO first before waiting period begins.
Long-Term Disability (LTD) All full-time employees of V-Soft are eligible to participate in Long-Term Disability the first of the month following the hire date. Company paid. Provided through Anthem Life. Pays 60% of your salary up to a maximum of $6000 per month. You must exhaust your STD benefits first. Benefit duration can last up to age 65.
401K All full-time employees of V-Soft, age 21 or older, are eligible to participate after six months of continuous employment. Tax-deferred retirement plan provided through ADP Retirement Services. Automatic enrollment of 3% after six month of services unless declined. Employer match is discretionary (percentage to be determined based on profitability at year-end) on all employee contributions up to 6%.
PTO (Paid Time Off) All full-time employees of V-Soft are eligible for PTO (Paid Time Off). PTO is accrued as follows:
0-6 months of services – 0 PTO days
6-12 months of services – 3 PTO days
12+ months of services – 1 day (8hrs) per month while on a billable project.
Holidays All full-time employees of V-Soft are immediately eligible for paid holidays. As per List of V-Soft’s 2012 Paid Holidays. Please note that employees who are at client location on an assignment will observe client-observed holidays. Client forced vacation or holidays will not be paid for.
Direct Deposit All full-time employees of V-Soft are eligible for direct deposit. Employees of V-Soft may have paycheck directly deposited into either a checking account or credit union account upon the completion of the Direct Deposit Enrollment Form (effective from second pay check). A voided or cancelled check must be included with the enrollment form.
Referal Bonus All employees of V- Soft are eligible for Employee Referral Bonus. An incentive of up to $500 is paid for any qualified referral resulting in professional-level hire.
Professional Development Plan A continuing employee whose start date falls between Jan 1 – Dec 31 is eligible for reimbursement on his/her anniversary  date of the following year. Employees can be reimbursed up to $500 in a calendar year for registration fees for conferences, workshops, training, seminars, and certifications (upon successful clearance). This benefits information presents highlights of some of V-Soft’s benefit program. V-Soft may make changes to the programs at any time and from time-to-time in its sole discretion, with or without advance notice. This is not intended to be a complete description or a summary of all the terms and conditions of program.

In addition, V-Soft provides visa and immigration assistance, such as H-1 to eligible employees. V-Soft also provides permanent residence (Green Card) application assistance to qualified and eligible employees.
